The London Stock Exchange issued a "report" on the causes of the technical problems that halted trading on April 5 for seven hours and 45 minutes, blaming a software fault. It became clear upon further investigation that human error had quite a lot to do with it, too. The exchange does not specify which bit of software was at fault, stating only that Andersen Consulting provided it and that it caused a delay in the overnight data processing, which ended up taking four hours.
